Before filling out a registration form, check you are in our catchment area.
The GPs at Mickleover Surgery feel it is not clinically appropriate or practical to register patients far away from their home address.
You can also find out which GPs you can register with on the NHS website.
Registering with the surgery
To register with the surgery:
- download and fill out a GMS1 and a new patient questionnaire (PDF) and bring them in to the surgery
- collect a registration pack from reception
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
The practice does not discriminate against race, gender, social class, age, religion, sexual orientation or appearance when considering registration applications.
Neither does the practice discriminate in any way against any disability or existing medical condition of any patient who may apply to join the practice list which is currently under review.
